Request Multiple Copies of Book

Over 3500 complimentary copies were mailed – mostly to podiatrists, orthopedists, physical therapists – in August 2010.  Anyone who is interested in buying the book in bulk may contact me through this form.  Please indicate how many books you would like and any other pertinent facts regarding your request.

Podiatrists, physical therapists, orthopedists, shoe store owners, and others who are best suited to review the book and/or display it in their offices are eligible to request a free copy of the book. There will be 1,000 free copies distributed. If you accept a free copy, please provide a review (On Amazon.com, for example.) – even if it is a bad review – and/or display the book in your waiting room or other prominent location.
